Healthy Families Massachusetts is a statewide home visiting program available for first-time parents under age 20 and is funded by the Children’s Trust Fund in partnership with the Massachusetts Department of Public Health. Parents are eligible for these services during pregnancy until their child is three years old. 

The program provides prenatal education, home visits, parenting activities, and support groups. It seeks to nurture, strengthen, and support families in their physical, emotional, and social development. This is accomplished with a cornerstone of home visitation, strong community collaboration, and a commitment to quality for enrolled families. Each family is provided with the tools to create their optimum healthy family by respecting their cultural beliefs, by valuing their diversity, and by building upon their inherent strengths.

Eligibility

•First-time young parents and their families.
•Parents and families do not have to use a Hallmark Health facility or physician.
•Parent must be 20 years or younger at the time of referral·
•Baby must be less than 1 year old at the time of referral·
•For Hallmark Health Healthy Families Program, client must live in “Service Area.”

Home visits are generally provided by a specially trained paraprofessional home visitor (background in health or human services, child care, plus life experience). A specially trained Healthy Families Pediatric/Maternal Child Health Home Care Nurse may provide short-term prenatal and postpartum visits.

Service Area

Parents must live in Everett, Malden, Medford, Melrose, North Reading, Reading, Stoneham, and Wakefield. If outside this area, please call 1-888-775-4KIDS to obtain the name of a Healthy Families provider.

Insurance

Most insurances cover 1-3 nursing visits with prior approval and medical necessity. Healthy Families covers home visits if not covered by insurance, and covers home visitor visits as well as group and educational programs.
