HALLMARK HEALTH VISITING NURSE ASSOCIATION

Quality Care and Service Excellence

Hallmark Health Visiting Nurse Association (HHVNA) has taken high tech equipment and a highly trained staff---once available only in a hospital---and brought them home. Through specialized programs, Hallmark Health VNA is on the forefront of innovation, developing new and exciting services to meet the individual needs of patients and families. Our goal is to improve the quality of life for our patients and their families by responding to their physical, emotional, and care needs.

When you choose Hallmark Health VNA, you are choosing Quality, Teamwork, and Service.

Quality through our commitment to deliver the very best care to our patients from a competent, caring staff.

Teamwork through our interdisciplinary team approach to care, which includes the patient and patient’s family as members of this team.

Service through commitment to treat our patients with the utmost respect and dignity and to provide them with the most appropriate level of care.

We provide our patients with experienced professionals and paraprofessionals across an entire range of home health care. Our team works closely with the patient’s primary physician, who remains active in the patient’s home care program. Patients and families are also at the heart of this team. Our goal is to always keep the lines of communication open and to involve our patients and their families in the decision-making process.

Our staff is selected on their competence and expertise, as well as their positive, interpersonal skills. All of our staff members undergo a rigorous screening process, including personal interviews, skills evaluation, professional references, and background checks.

Hallmark Health VNA is accredited by the Joint Commission Accreditation of Health Care Organizations and certified by the Massachusetts Department of Public Health. HHVNA continuously monitors and evaluates the quality and appropriateness of services, using measurable standards and outcome-based goals.
