REFERRAL INFORMATION

Admission Criteria

Home evaluation can be done without a physician’s order. However, before services begin, our staff must obtain a physician’s order.

Patients do not have to be under the care of a Hallmark Health physician or utilize a Hallmark Health facility. We accept referrals from a variety of sources, including individuals, families, friends, physicians, discharge planners, and community agencies.

Insurance

Most home health care services are covered by Medicare, Medicaid, HMOs, Managed Care Organizations, Veteran’s Administration, and other health plans. Our staff will contact your insurance company and help determine which services are covered by your policy. Bills will be sent directly to your insurance company.

Hallmark Health VNA and Hospice, Inc. also offers a private pay program, Best Care, for those who choose additional care not covered by insurance.

Hallmark Health Home Care Service Area includes but is not limited to: Arlington, Burlington, Cambridge, Charlestown, Chelsea, East Boston, Everett, Lynn, Lynnfield, Malden, Medford, Melrose, North Reading, Reading, Revere, Saugus, Somerville, Stoneham, Wakefield, Winchester, Winthrop, Woburn.

Making a Referral

To refer, please call our Intake Department at (781)338-7900. When calling, it would be helpful to have the following information available

  • Name
  • Address, Apartment Number
  • Telephone Number
  • Date of Birth
  • Social Security Number
  • Diagnosis
  • Medications
  • Insurance Information
  • Physician Name and Telephone
  • Treatment Plan or Services requested

Physician orders are required before any treatment is provided.
