Hallmark Health Hospice provides a full range of services tailored to meet the needs of each patient. Hospice team members are just a telephone call away, 24 hours a day, for support, consultation, and home visits when needed.
Hallmark Health Hospice provides care in the patient’s primary residence, either the home or nursing home, and provides in-patient care on a short-term basis to address symptom management. Hallmark Health Hospice has contracts with several long-term care facilities and area hospitals.

Our nurses are specially trained in managing the symptoms that accompany a serious illness and are experts at managing pain. They work with other members of the Hallmark Health Hospice team to offer comprehensive services that best meet the patient’s needs.
Hospice social workers and bereavement counselors have advanced education in counseling and helping individuals and families cope with the emotional, social, and financial stresses associated with life-limiting illness. Social Workers and Counselors make scheduled visits as needed and are available for emergency visits or consultation.
Certified Home Health Aides bring gentleness and warmth as they offer assistance with personal care in a manner mindful of our patient's dignity and independence. Our home health aides have special training in caring for those with life-limiting illnesses.
Bereavement support groups are offered on an ongoing basis and provide support to those who have lost a loved one. Experienced bereavement counselors coordinate these groups.
Hallmark Health Visiting Nurse Association and Hospice, Inc. provides bereavement support to children ages 5 to 13 who have lost a loved one. Find out More
Our specially trained volunteers offer loving hands and listening ears to meet special needs of patients and families. Hospice volunteers may temporarily relieve caregivers giving them time to go out or to rest. Hospice provides volunteer training and recognition.
Hallmark Health Hospice provides, upon request, spiritual counseling and support for patients and family members. Spiritual care is nondenominational and does not impose any religious or philosophical beliefs. A Memorial Services is held annually and is open to the public.
Research has shown that utilizing alternative therapies contributes to patient comfort and quality of life. To provide patients with additional symptom management, Hallmark Health Hospice contracts with the Huggins Center for Alternative Therapies, located in Melrose, Massachusetts. These services are offered through the generous donations from families and friends, whose loved ones have received care from Hallmark Health Hospice. Alternative therapies offered include: Massage, Reiki, Foot Reflexology, Acupuncture, Polarity, and Shiatsu.
Other therapies and services (when indicated) include: physical, occupational, and speech therapies; nutritional counseling.
For those with a life-limiting illness who are not psychologically or medically ready for hospice, Hallmark Health Visiting Nurse Association offers a special program known as The Bridge/Palliative Care Program. In this home health program, patients are assigned to a primary nurse and, if needed, a social worker and home health aide specially trained in palliative care. If the patient elects the Hospice benefit, The Bridge/Palliative Care Program provides a seamless transition of care at a time when support and continuity of care is most important to the patient and family. |
