REFERRAL INFORMATION

Eligibility and Coverage
Hospice services are available to any person who is diagnosed with a disease that is expected to limit his or her life span to six months or less. We accept eligible patients with any life-limiting illness regardless of their ability to pay. Our patients are of all ages, races, and religions.

At any time during a life-limiting illness, it is appropriate to discuss all of the patient’s options, including hospice. The decision always belongs to the patient. A patient’s final days, weeks, or months are more comfortable and satisfying when hospice care and support are offered as early as possible.

Hospice is covered by Medicare, Medicaid, and by most private insurance policies. Charitable contributions from individuals, businesses, and organizations assist in providing additional services not covered by the patient’s insurance.

Physicians, hospitals, families, and friends can make a referral.

We provide hospice care services to patients and families from cities and towns north of Boston. Our service area includes, but is not limited to

Arlington, Burlington, Cambridge, Charlestown, Chelsea, East Boston, Everett, Lynn, Lynnfield, Malden, Medford, Melrose, North Reading, Reading, Revere, Saugus, Somerville, Stoneham, Wakefield, Winchester, Winthrop, Woburn.
