The following people are eligible for participation in the Healthy Families program:

• First-time young parents and their families. Parents and families do not have to use a Hallmark Health physician or facility
• Parent(s) must be 20 years or younger at the time of referral
• Baby must be less than one year old at the time of referral
• Participant must live in service area listed under "Service Area"
